Looking for Terry Lee Wulff

Hi. BM2/CTO1 Gerald Condell, 67/96. Looking for any information, details, ship logs about shipmate Terry Lee Wulff who died after attempting to dive off the ship, ended up heading his head on the deck. He later died at the hospital two days later. Any and all information will greatly be appreciated.
